Process Measures
The program was planned and implemented well. The planning of the initiative had well-defined objectives, which was to reduce the chances of avoidable hospitalizations in the selected population. The planning also included an objective of educating patients, their households, and the community on prevention of fall and UTI prevention strategies. All the resources were also availed to prepare for the implementation of the program. These resources included gas money, puzzles, gel pens, and coloring books, among other materials. Besides, the implementation was also conducted well. Accordingly, the program was implemented for an appropriate population (“adults who are aged 65 years and older with a diagnosis of Alzheimer’s disease and other dementias”), who needed the minimization of the chances of avoidable hospitalizations (Phelan, Debnam, Anderson, & Owens, 2015). Similarly, the most affected group contributed to the planning, implementation, and evaluation of the initiative. For example, the program involved spending time with the administrator of and people residing in the assisted living system to identify their needs. The caregivers were also engaged in the program to enhance the provision of care to the clients. The engagement of the participants and successful implementation of the program objectives imply that the participants were, to the large extent, satisfied with the program.

The participants are said to have high chances of hospitalization due to the fall. As a result, successfully implementing a program that will reduce such risks meant that they were fully satisfied with it.
Outcome Measures
The program attained its objectives well. The two primary objectives of reducing the risk of avoidable hospitalizations in the targeted population, as well as educating patients, their households, and the community of the fall prevention techniques, were achieved. All the risks were identified and ways of reducing theme were highlighted for the selected population. The number of individual hospitalized significantly reduced. Moreover, the participants, including the patients and their families, were largely impacted by the program in that they became well-educated on the inevitable cause of the hospitalization and reduced levels of preventable hospitalization with regards to the fall risks. Finally, the community also realized the need to remain supportive of the stakeholders, patients, and family members of those affected by the health challenge of UTI and falls (Phelan et al. 2015). The community was also informed of the ways of avoiding the risks of falls and UTIs. Nevertheless, it can be said that there were no unintended consequences observed in this project as the realized outcomes were within the purpose or objectives of the initiative.
