Problem Statement
There has been an increase in the number of errors committed by health practitioners. This leads to the undermining of the quality of healthcare as medical errors are costly both to the hospitals and the actual patient.
Background: Why is this issue a problem?
The medical errors show that the system is not efficient enough to curb the errors from happening. The repeating of medical errors portrays a picture of negligence to the patients hence may lead to costly litigations.
Landscape: Identify stakeholders and their key factors.
Key stakeholders consist of healthcare providers and hospitals. Each stakeholder has direct key factors that affect how they function. Healthcare providers key factors are a diagnosis of the patient correctly and lack of adequate information. Hospitals key factors are litigations and punishment to the medical practitioners.
Options
Option 1: monitor and report medical errors hence can find out the root-cause problem and put in place plans to avoid similar incidents in the future (Tietelbaum & Wilensky,2007).
Option 2: Creation of informatics that combines computer technology with nursing hence better communication, documentation, and efficiency
Option 3: Establishing of an inter-professional team in that one patient is treated by a number of specialists to reduce chances of error.
Recommendation
Option 1: Analyses of root-cause problems
The health providers should maintain a record of the medical errors in order to analyses the reports and know the root-cause of the errors.

The root-cause analysis is used to show trends over a period of time hence it’s easier to identify the link that leads to medical errors hence implementing systems that can avoid the error in the future. The systems however should not remove the freedom that the health providers have in dealing with patients for fear or repercussion.
