Consent Form
A consent form is a legally binding document that shows a participant is willing to partake in a study. Furthermore, it proves they agreed to the probable the risks associated with the study. Participants receive a form to sign after agreeing to the set terms and conditions and the outlined risks and costs. This form is the consent form. They are, however, allowed to quit the study at any point of the study if they so desire. These form’s utility arises when a person starts treatment with a psychologist or when research is being conducted on human subjects.
To conduct a study, there are two significant aspects to consider. These are the consent form and the participant information sheets (PIS). The participant information sheets are a broad overview of what the study entails the participant requirements, the risks and costs. The consent form highlights the key points contained in the participant information sheets and a section where the participant is required to sign.
The basic information in a PIS are the heading of the research study, an introductory paragraph of the study, the aim of the study and why a particular participant is deemed fit for the study. Moreover, a section of the PIS outlines that taking part in the study is voluntary, the proceedings, venues, duration, expected outcomes of each assessments are outlined. Furthermore, the tasks of the participants, the risks they will be exposed, likely benefits to be accrued are outlined.

In addition, the participant is guaranteed of privacy and he or she is told how the outcomes will be useful to the researchers. The organizations providing the capital and those that conducted the ethical appraisals of the study will be provided. The participant provides his contact information (University of Cambridge).
Consequently, a consent form contains only the principle facts contained in a PIS summarized into brief statements. The participant has the choice of either assenting or dissenting with the terms provided before signing up for the study. These statements are answered in a yes/no format. Debriefing Form
A debriefing form is part of the research process used when the participants might be deceived in studies that require deception. The participant is informed of the research hypothesis, why it is necessary for them to be deceived and the process of deception. Therefore, a participant has to fill a debriefing statement as proof of their informing.
