Wellness programs are activities that patients undertake under the supervision of a caregiver such as physical exercise, dieting, medical checks; that aims at improving their health conditions live healthy and happy lifestyles. (Winston, 2013). In Valdosta, GA, there are various community resources available for patients living with disability. A good example is the PA, an elderly patient, 68 years old, succumbed to an accident making him unable to walk on foot and do other important vital normal human activities like bathing, dressing, cooking among others. In such a case, a wellness program can be developed by using the resources within the community to assist the patient to live a healthy and happy lifestyle.
According to the ‘department of health and human services,’ resources such as information and assistance centers will enable older patient living with disability get information, counseling programs as well as access to long term support. Other community resources such as aging and disability resource centers, area agencies on aging, health insurance counseling, elder abuse prevention services, legal services, as well as, long term care Ombudsman. Health and insurance resources involve professionals and volunteers that offer unbiased, one on one counseling to help the disabled and older people like PA understand medical insurance as well as their benefits. Elderly abuse prevention services involve specialists that investigate abuse, neglect and exploitation of the elderly like PA.

The long term Ombudsman resources encompass advocates for nursing facilities, care homes and similar adult and disability care facilities. They aim at ensuring legal procedures are met in improving the lifestyle quality of adults and disabled people. These resources enhance the optimum level of patient PA well-being making him live healthy and happy lifestyles despite his condition.
